HIPEC – Hyperthermic Intraperitoneal Chemotherapy

For patients whose gastrointestinal, ovarian, or appendiceal cancer has spread within the abdominal cavity — a pattern that leaves few options with standard intravenous chemotherapy — Sheba offers hyperthermic intraperitoneal chemotherapy (HIPEC), a combined surgical and chemotherapy technique. Hundreds of patients have already undergone the treatment at Sheba.

HIPEC follows cytoreductive surgery (CRS), in which all visible tumors and lesions are surgically removed. Because CRS cannot remove microscopic remnants of cancer, the abdominal cavity is then bathed for 60–90 minutes in a concentrated chemotherapy “wash” heated to 42°C (107.6°F) — a dose up to 20 times higher than standard IV chemotherapy can safely deliver, made possible because the drug is applied directly rather than through the bloodstream. A laparoscopic, minimally invasive approach to HIPEC is also available at Sheba, allowing eligible patients to leave the hospital after just a few days without surgical drains.

“HIPEC was originally used only for cancer of the appendix and mesothelioma, two relatively rare diseases. However, in the last decade HIPEC has been applied to metastatic cancer in the abdominal cavity originating in the colon, stomach, and ovaries,” says Prof. Aviram Nissan, Director of the Department of Surgery C at Sheba.

Who Is a Candidate for HIPEC?

  • Metastatic cancer confined to the peritoneal cavity, without spread to distant organs, confirmed by chest/abdomen CT or PET-CT
  • Good functional status, free of significant kidney or cardiovascular disease

Without treatment, patients with extensive abdominal-cavity tumor spread typically survive 5–12 months. Published outcomes show HIPEC, combined with complete surgical removal of visible tumors, substantially outperforming conventional chemotherapy or surgery alone — giving patients previously considered incurable a realistic chance at extending their life expectancy. Because of the complexity of the surgery, Sheba emphasizes that both the pre-operative assessment and the procedure itself are carried out by a team specializing specifically in this technique.

CAR-T Cell Therapy for Leukemia and Lymphoma

CAR T-cell therapy reprograms a patient’s own white blood cells to hunt down and kill cancer cells. Sheba performs the entire process on-site: white blood cells are collected from the patient, sent to a specialized laboratory where they are engineered to express chimeric antigen receptors (CARs) targeting the patient’s specific cancer, multiplied to millions of cells, and then infused back into the bloodstream — where the modified cells continue attacking the tumor for months or even years after a single treatment.

Who Is a Candidate?

CAR-T is provided to leukemia and lymphoma patients who have undergone at least two previous treatments. At Sheba, therapy is offered for acute lymphocytic leukemia (ALL) in adults and children, non-Hodgkin lymphoma (NHL) in adults and children, and multiple myeloma.

Outcomes

CAR-T has been especially effective in younger patients: young children have achieved a total recovery rate of about 85–90%, and roughly 80% of teenagers receiving the treatment have made a complete recovery from their leukemia. Across the total patient population, the success rate is approximately 70%.

Unlike conventional chemotherapy, CAR-T is given as a single infusion rather than repeated sessions, so most patients avoid the nausea, vomiting, and hair loss typically associated with chemotherapy. Serious complications, when they occur, are usually temporary, appear within the first one to two weeks after treatment, and are managed with medication by Sheba’s specialized team.

Care is led by Prof. Arnon Nagler and Dr. Abraham Avigdor of the Division of Hematology and Bone Marrow Transplantation for adult patients, and by Prof. Amos Toren and Dr. Elad Jacoby of the Pediatric Hemato-Oncology and BMT Department for children.


CAR-T Cell Therapy for Autoimmune Disease

Sheba is also translating CAR-T’s success in oncology into a pioneering research program for severe, treatment-resistant autoimmune conditions, including systemic lupus erythematosus, rheumatoid arthritis, Sjögren’s syndrome, scleroderma, dermatomyositis, and vasculitis.

This is a research and compassionate-use program, not a routine or broadly available treatment. Beyond formal clinical trials, Sheba has developed a self-manufactured CAR T-cell therapy offered under compassionate use for the most critical cases — a program the hospital describes as unique worldwide. It has already treated patients with advanced rheumatoid arthritis who had exhausted all available biologic treatments and were severely disabled; these patients showed strong clinical results, documented in a published case report. Sheba was also selected, alongside other leading Israeli medical centers, to participate in Israel’s national CAR-T initiative, and is the country’s leading rheumatology clinical-trial hub.

Because this program treats the most severe, refractory cases on a compassionate-use and research basis, eligibility is evaluated individually by Sheba’s clinical team following review of a patient’s full treatment history.

Sheba is a leader in TIL immunotherapy, in which a portion of the patient’s tumor is removed, its infiltrating lymphocytes are isolated and grown to very large numbers in Sheba’s own laboratory, and then reintroduced into the patient’s body to hunt down tumor cells. About 50% of treated patients show improvement, and approximately 25% are cured of their melanoma. Sheba’s doctors have participated in TIL research from its earliest stages, and every part of the process — including the lymphocyte genetic engineering — is performed in-house at a single facility.

“Usually, within 4 weeks, we see the beginning of the clinical results. In the CT scans, you see the metastasis getting smaller and smaller, and some of the metastasis finally disappear. If you achieve complete response, it’s durable, and you actually cure patients with advanced melanoma,” says Prof. Jacob Schachter, Director Emeritus of the Ella Lemelbaum Institute for Immuno-Oncology.

Two additional therapies are available for patients who have not responded to immunotherapy. Fecal microbiota transplant introduces bacteria from melanoma patients who have responded to treatment into patients who have not, on the premise that changing the composition of the digestive tract’s bacteria can make a previously non-responsive patient receptive to immunotherapy. Immunopheresis uses a specialized filter to selectively remove immune-suppressive cytokines produced by cancer tumors, aiming to neutralize the tumor’s ability to block the patient’s natural immune defenses; unlike drug-based therapies, it is a “subtractive” approach intended to avoid added toxicity and side effects. Both treatments remain highly specialized and are offered at only a small number of medical institutions worldwide.
